Locum Band 7 Complex Discharge Occupational Therapist

JustOT is currently recruiting for an experienced Band 7 Complex Discharge Occupational Therapist to join a specialist team in West Wales.

This is an excellent opportunity for a confident senior OT to play a key role in facilitating safe, timely and effective discharges from an acute hospital setting, supporting highly vulnerable patients with complex needs.

The Role

You will work within a specialist team focused on early intervention, admission avoidance and complex discharge planning. The caseload will include frail and elderly patients, as well as individuals with respiratory conditions and stroke.

Key responsibilities include:

• Completing specialist occupational therapy assessments and delivering tailored interventions.
• Supporting complex discharge planning for patients with multiple needs.
• Facilitating early intervention to prevent unnecessary hospital admissions.
• Working collaboratively with the multidisciplinary team to ensure patients receive the right care, in the right place, at the right time.
• Supporting patients to maintain existing care packages wherever possible and reducing the need for additional support.
• Promoting positive clinical outcomes while improving discharge efficiency and reducing avoidable costs.

Working Hours

• Hours: 37.5 hours per week
• Shift pattern: Monday to Friday, 8:00am–4:00pm

Requirements

• HCPC registration as an Occupational Therapist.
• Previous NHS experience at Band 7 level or equivalent senior clinical experience.
• Strong experience in complex discharge planning and acute hospital settings.
• Experience working with frail and elderly patients, stroke and/or respiratory conditions.
• Excellent communication, clinical reasoning and multidisciplinary working skills.
