Locum B6 Community Reablement Occupational Therapist – Norfolk
JustOT are recruiting for a B6 Community Reablement OT to join a service in Norfolk.
This role will involve:
- Assessing patients and prescribing appropriate care packages
- Supporting patients with long-term conditions, frailty, palliative care and rehabilitation needs
- Promoting independence and quality of life
- Supporting early hospital discharge and reablement
- Working within an integrated MDT to prevent unnecessary hospital admissions
- Liaising with health professionals and statutory/non-statutory agencies to provide coordinated care
Requirements:
- B6 Occupational Therapy experience
- Community/reablement experience
- HCPC registration
- Full UK driving licence and access to your own vehicle
