Our client is looking for a newly graduate occupational therapist to work on a community based. Our client Connect2Care is a registered National Disability Insurance Scheme (NDIS) health provider, providing a range of community-based, allied health services to people of all ages and abilities across Australia. Services are delivered through multiple delivery models (including telehealth, community visits, and office-based services), and referrals may come through the NDIS, Medicare or privately. The Connect2Care team is a wonderful group of like-minded individuals working together to fulfil the same mission: to empower people to reach their personal goals.

Job Role:

• Using evidence-based and best practices to assess and provide interventions for children and adults living with a range of abilities to maximise function, capacity, and social engagement.

• Work with a varied caseload. This may include working with different ages (across the lifespan) and presentations, such as Autism Spectrum Disorder (ASD), Intellectual Disability (ID), Psychosocial Disability and Physical Disabilities or clients with a range of communication needs and culturally diverse backgrounds.

• Assisting clients with assistive technology assessment, recommendations, and applications (in relevant fields)

• Working within different funding schemes and models. Predominantly within the National Disability Insurance Scheme (NDIS), but can include Enhanced Primary Care (EPC), private etc.

• Working in different settings. The role is predominately mobile, travelling to designated locations to provide intervention in community settings, but can also include office and telehealth settings and times when one may decide to work from home.

• Meeting company professional expectations

o Invoicing accuracy: appropriate data entry to reflect the work schedule.

o Billable hours: achieving an average of 5 billable hours a day.

o Case list management: ensuring all clients have ongoing appointments (where appropriate), active plans and appropriate use of funding.

o Communication (internal and external): timely and transparent communication with key stakeholders

o Documentation: timely and accurate completion of report writing, case notes and correspondence

• Attending meetings, supervision and PD as required

o Punctual to meetings

o Active participation and engagement

• Building relationships with key stakeholders associated with Connect2Care's business, including therapists, clients, and referral sources.

• Understanding of the NDIS, expectations of being a provider and complying with the NDIS Code of Conduct

• Understating and adhering to Connect2Care's policies and procedures

Person Specification Essential Qualifications

• Relevant Tertiary Qualification

• First Aid (desirable, EP required)

• NDIS Worker Orientation Module

• Connect2Care Induction

Essential Requirements

• Registration with the relevant professional body

• Working with Children Check (as per relevant state)

• NDIS Worker Screening Check

• Evidence of COVID-19 vaccination (or valid exemption)

• Police Check and International Police Check (only applicable to individuals who have lived overseas)

• Driver's license and access to a reliable vehicle

• Car insurance

• Professional Indemnity and Public Liability insurance

• Appropriate working rights to work in Australia.
