•To work alongside people with a wide range of disabilities including, physical, learning and mental health disabilities, undertaking strengths-based functional assessments using most appropriate means and setting such as home visit, clinic assessment or virtual assessment. The post holder will be responsible for undertaking specialist functional assessments to consider:

•rehab approaches that will improve the person’s independence

•review of existing equipment and environment, to ensure the most appropriate and enabling equipment is in use

• identify interventions that empower the person and their carer in the least restrictive way.

• provide recommendations with a comprehensive risk-assessed approach, working to enable and optimise outcomes for residents and carers

• people’s needs in accordance with current legislation and anti-discriminatory practice, ensuring demonstrable person-centred and strengths-based outcomes

• carer’s needs including undertaking carers needs assessments when required, in line with the Care Act.

• To undertake: environmental ergonomic assessment of the person’s environment; assessment of cognitive and functional ability to carry out daily living tasks; assessment of moving and handling situations.

• To be responsible for providing specialist occupational therapy support to Adult Social Care staff, citizens, formal and informal carers (including carer assessments).

• To contribute to and participate in project work and the roll out of service developments including audits, liaisons, working groups and reviews. To embrace current good practice as business as usual, to ensure maximum independence and optimized care outcomes. To attend any necessary training to support knowledge and skill base required for this post

• To take and act upon new referrals. To support workflow through timely completion of work and high standards of recording.

• To undertake specialist moving and handling risk assessments, advising staff, citizens and carers on techniques, equipment and methods of optimizing care and reducing risk and provide a clear and appropriate moving and handling plans.

• To discuss with people an outcome focused plan arising from their assessment, identifying the choices and options to meet their needs and maximising their independence, ensuring at all times that value for money is achieved.
