JustOT are recruiting for an experienced Band 6 Community Rapid Response / Admission Avoidance Occupational Therapist to join a dedicated NHS community team in Essex.

This is an excellent opportunity for an autonomous OT with community experience who thrives in a fast-paced environment, helping to prevent unnecessary hospital admissions and supporting patients to remain safely in their own homes.

The Role

• Provide urgent occupational therapy assessments and interventions to prevent hospital admissions.
• Deliver rapid response support to individuals experiencing acute functional decline.
• Complete functional, environmental and falls risk assessments within patients' homes.
• Recommend and provide equipment and immediate interventions to promote safe independence.
• Work collaboratively within a multidisciplinary team to deliver crisis intervention and urgent care planning.
• Support safe discharge pathways and help reduce delays in transfer of care.
• Liaise with health, social care and voluntary sector services to coordinate ongoing community support.
• Develop person-centred care plans that maximise independence and reduce admission risk.
• Monitor patient progress, review interventions and maintain accurate clinical records.

Requirements

• HCPC registered Occupational Therapist.
• Previous NHS experience as a Band 6 Occupational Therapist.
• Community and Rapid Response/Admission Avoidance experience is highly desirable.
• Strong assessment, clinical reasoning and caseload management skills.
• Ability to work independently in the community.
• Full UK driving licence and access to a vehicle is essential.
