This is an excellent opportunity for an experienced and motivated Occupational Therapist to join our Prevention & Early Intervention service.

You will play a key role in supporting adults (aged 18+) with physical disabilities that impact functional ability, using a person-centred, strength-based approach to promote independence, wellbeing, and meaningful engagement in daily life. The focus of this role is to reduce risks (including falls), prevent escalation, and avoid outcomes such as hospital admissions, formal care packages, and functional decline.

Working autonomously within a supportive team, you will manage a diverse and complex caseload, delivering high-quality OT interventions across the community.

Key Responsibilities

 Provide high-quality OT assessments with a clear person-centred and strength-based focus

Use strong clinical knowledge of physical health conditions to guide assessment and intervention

Manage and prioritise a varied caseload independently, with access to supervision where needed

Assess for and arrange minor equipment, specialist equipment, and major adaptations (including Disabled Facilities Grants)

Complete manual handling assessments and provide advice

Contribute to rehousing reports and provide effective signposting to appropriate services

Maintain clear, timely, and comprehensive documentation

Work collaboratively with individuals, carers, and multidisciplinary colleagues

Apply a strong understanding of safeguarding and the Care Act

Essential / Desirable Requirements:

Previous community social care OT experience highly desirable

At least 2 years post-qualifying experience
