We are currently recruiting for a Community Occupational Therapist to provide high-quality Occupational Therapy services within the community.
The successful candidate will work with a multicultural and diverse population, delivering person-centred care to patients and their carers with a range of complex physical and psychological conditions. The role involves carrying out comprehensive Occupational Therapy assessments, developing clinical diagnoses and delivering individualised treatment programmes to support independence and wellbeing.
This is an excellent opportunity for an experienced Occupational Therapist who is passionate about community-based rehabilitation and providing holistic care within a multidisciplinary environment.
Key Responsibilities
- Deliver a high standard of Occupational Therapy care within the community
- Undertake comprehensive Occupational Therapy assessments for patients with complex physical and psychological conditions
- Develop clinical diagnoses based on assessment findings
- Plan, implement and evaluate individualised treatment programmes
- Work closely with patients, carers and multidisciplinary teams to support rehabilitation goals
- Promote independence, safety and wellbeing within the community setting
- Maintain accurate and timely clinical documentation
- Manage and prioritise a varied caseload effectively
- Provide advice, education and support to patients and carers
Essential Requirements
- HCPC registered Occupational Therapist
- Previous experience working within a community setting
- Experience managing patients with complex physical and psychological presentations
- Excellent assessment, clinical reasoning and treatment planning skills
- Strong communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Must be a car driver or have reliable means of transport to travel across the borough
