Rate: £27.13 per hour
Working Pattern: Full time, 36 Hours

Job Description:
Integrated Triage Team - is a multi-disciplinary Team that offers initial triage and response to health and social care needs of patients within primary and secondary care. The team provides holistic assessment of needs within the patient's home and over the telephone.
We will look to you to provide a professional and personalised occupational therapy service to adults and elderly residents with physical disability and dual diagnosis of physical disability and mental health issues. The successful candidate will assess each individual case and recommend rehabilitative solutions that promote independence and improve the individuals' quality of life, wherever possible.
You will also take responsibility for shaping the occupational therapy service by continuously evaluating it on a professional and administrative level and providing recommendations for improvement.
You'll work closely with Nurses, Social Workers, Rehabilitation Assistants, Physiotherapists, Doctors, and volunteers from both Health and Social Care backgrounds.
We will look to you to play a key role in the assessment and treatment of people, caseload management, supervision and training of junior staff as appropriate, complete Access / Home visits, support other Team members as appropriate and attend family and MDT meetings.
Due to the fact that you'll be carrying out visits in patients' homes and other settings within the community, it is essential that you are able to travel around the Borough.
