Working Hours: 30 hours per week
Based on site & community

Job Description:
It would entail inpatient rehab, facilitating discharge home and then possibly following the person out into the community if required.
This may require working with the reablement team to assess their needs at home and right sizing their care.
Experience of physical inpatient work and ideally community work who has a good knowledge of manual handling, postural seating assessments and assistive aids/adaptations.
