Working Hours: Part time 22.5 hours per week - 3 days

Job Description:
The role is based at Chepstow Community Hospital, with responsibilities extending into the community, supporting individuals within their own homes. The postholder will carry out occupational therapy assessments and interventions across a range of areas. the role includes participation in a Duty rota, one day a week.
The hours of work are generally 9am to 5pm, 3 days a week.

Deliver occupational therapy interventions in patients' homes and community settings, supporting independence and preventing unnecessary hospital admissions.
Conduct comprehensive assessments and develop tailored intervention plans for individuals with long-term complex needs.
Support admission avoidance through timely and effective therapeutic input
Recommend and arrange assistive equipment and home adaptations, including processing Disabled Facilities Grants (DFGs)
Conduct housing assessments and contribute to manual handling and reablement plans
Participate in a weekly Duty rota, responding to queries and undertaking urgent visits.
Act as a Trusted Assessor to set up care packages based on assessed needs and complete reviews of existing care arrangements.
Manage a varied caseload efficiently, demonstrating strong organisational and clinical reasoning skills.
