JustOT are looking to recruit a band 6 Home Treatment Team Occupational Therapist for a locum position with our client in London.

The pay for this position is £22- £26 per hour, and we are looking for someone who can start as soon as possible for an ongoing duration.

The service places the user at the heart of the care planning process, and aims to promote independence, deliver outcome focused care, and provide a seamless journey through the care system. The service will have a single point of access and use a multi-disciplinary assessment and care planning framework. The service will respond swiftly to service user need and, where possible, will treat people in their own home or normal place of residence.
JOB SUMMARY:

* Provide assessment and review for those following discharge from out of area hospitals returning home to Hackney and the City of London and those seen by Rapid Care to check functional ability and ensure safety at home including anticipatory oversight of need, scrutiny of discharge arrangements and promotion of single-handed care as part of the Discharge to Assess process
