JustOT are looking to recruit a band 6 Community Reablement Social Services Occupational Therapist for a locum position with our client in Worcester.

The pay for this position is £29.56 per hour, and we are looking for someone who can start as soon as possible for an ongoing duration.

We are looking for an OT to join our Reablement Team focusing on their new Community Reablement project; assisting patients requiring additional support and rehabilitation who need increased care or therapy input following referrals initially made through to the area social work teams. Assessments are holistic and focused on maximising independence at every stage. Patients are at the heart of everything we do As a member of the reablement team, you will work closely with therapists and social care teams.
You will perform holistic, goal focused, patient centred assessments, and develop thorough treatment programmes to meet the goals. You will manage a varied caseload, spanning all Occupational Therapy specialities and in addition look at care minimilisation.
You will work closely with both health and social care colleagues, in a blended approach to patient care. Physiotherapists, nurses, occupational therapists, therapy assistants and social care staff undertake collaborative working on a daily basis to ensure that all patient needs are met.
* Undertake initial assessments focusing on both Occupational Therapy and care need aspects and provide therapeutic intervention for a designated caseload, working closely with individuals to identify goals as part of their overall care plan with the aim of increasing independence and minimising care requirements.
* Provide case management support for a designated caseload, with guidance from the Senior Occupational Therapist. * Develop and co-ordinate individualised rehabilitation plans in close liaison with the team, individuals, their carers as well as all other services.
* Enable individuals to achieve maximum independence with activities of daily living, providing appropriate direction and guidance to Therapy/Promoting Independence assistants in the service to support an individual's rehabilitation and reablement where able.
* This post will mainly cover the Wychavon area of the county as well as supporting when required within the Wyre Forest area. These areas have been specifically chosen for the project. Unless requiring urgent input you will be able to work autonomously to plan your assessments/reviews over the course of the week.
Successful candidates must hold a degree in occupational therapy and be HCPC registered.
