JustOT are recruiting for an experienced, HCPC registered, Occupational therapist to work with our well-respected client based in south west London .

The successful candidate will have experience in

Response and Rehab Team

Team description/dynamic: The purpose of the Richmond Response & Rehabilitation Team is to work with clients, carers and rehab support workers to maximise clients physical and social functioning to enable them to remain within the community for as long as possible. The multi-disciplinary team (OT, Physio, Nurse, SW, Therapy assistant, Dietician) undertake elements of hospital discharge work including hospital liaison, case finding and Discharge to Assess. We provide a Rapid response service to prevent unnecessary hospital admission and record client observations(NEWS2 National Early Warning Score) to assess client's clinical presentation and need for escalation. We provide therapy, equipment and/or package of Reablement support to prevent hospital admission.
The aim of the service is to reduce inappropriate hospital admission, minimise hospital length of stay and support vulnerable clients at home, this Occupational Therapy role works within a multi-disciplinary, multi-agency team and in partnership with primary health care and social care professionals, providing expert case management, clinical assessment, and creating individualised client centred goals, promoting self-management.
The post holder will be based in the community.

Hourly Umbrella Pay Rate: £28

Expected Assignment Duration: Ongoing

Case Load: Approx 15 patients on caseload. 4 new referrals per we
