ASSISTANT TEAM MANAGER - OCCUPATIONAL THERAPIST REABLEMENT TEAM - MUST HAVE EXPERIENCE/BACKGROUND IN OCCUPATIONAL THERAPY
specifically, for a locality hub manager for reablement service for adults within occupational therapy.
Locality Hub provides the triage service for referrals for customers on the "Home" pathway from the Acute Hospitals (Great Western Hospital, Salisbury Foundation Trust and Royal United Hospital Bath) and the Community Hospitals in Wiltshire.
Referrals in the hub are triaged in partnership with our Health Colleagues and an agreed route for the facilitation for discharge agreed between the Health Home First Teams and Wiltshire Council Reablement. Wiltshire Council Reablement has three operational therapy teams that cover the County, North, West and South, and a reablement provider service with support staff across county.
Currently the managers and advanced practitioners share responsibility for the triaging of the referrals and the successful candidate will take on the principal responsibility for this with support from the Reablement Managers.
The service runs 7 days a week and there is a rota to cover these hours, there is an expectation that the candidate will work full time hours over five days one of these five days will be a Saturday or Sunday. In addition to undertaking the triaging role the successful candidate will also be responsible for supporting the Occupational Therapists who are working in the community out of hours including weekends.
This will include professional support and guidance, responsibly for escalating any lone working situations and advising the reablement staff where a customer's safety and well-being is at risk.
Pay Rate: £47.45 per hour
