JustOT are recruiting for a Band 6 Occupational Therapist who is looking to work in the Buckinghamshire region.

Position: Community Discharge
Start Date: ASAP
Duration: 3 months
Full time: 37.5 hours a week

This role will follow up patients at home the day after discharge from hospital.

The post holder will be responsible for proactively triaging and accepting referrals into the Community services this will involve facilitating discharges from acute services, planning and enabling the pathways of referrals and determining the most appropriate pathway into- community services.

The post holder will be a registered clinician who will liaise with professional staff within designated clinical areas with the overall aim of preventing delays. The post holder will be proactive in generating solutions to implement appropriate discharges in a safe and timely manner, ensuring the client receives continuity of care through clear and timely communication with all involved in their care.

The post holder will have experienced Telephone triage assessment skills and knowledge of the services included.
