Our client is looking for a Band 5 Occupatioanle Therapist to join their team .
You will be required to be part of the therapy department to provide an occupational therapy service to residents at the same time working within the broader multi-disciplinary team.
DUTIES AND RESPONSIBILITIES
1.As part of the Occupational therapy team, to assess new residents on admission for health and wellbeing needs.
2.Plan and implement occupational therapy interventions/treatments following needs assessment of physiological and psychological functioning.
3.Review and re-assess residents' needs following internal bedroom transfers, change of medical status, annual reviews or other.
4.Encourage and empower residents to realize and maintain their potential and sense of achievement through provision of aids and equipment, practice, treatment and meaningful engagement.
5.As a significant part of the job role, to implement, develop and lead on the 'Living well with Activity Toolkit' and 'PAL' assessment on units. Record and present findings. Motivate and facilitate staff groups. Work with the MDT to imbed this activity culture.
6.To carry out checks on equipment and activities using the audit tools.
7.Provide advice, training and practical demonstration to staff, residents and relatives relevant to occupational therapy within the scope of knowledge
8.Carry out comprehensive falls assessments as part of therapy team falls prevention programme
9.Carry out environmental assessments for new admissions and internal transfers
10.Carry out seating assessments and postural adjustments for wheelchairs and static chairs
11.To demonstrate person centered care.
12.Record interventions and respond to referrals in accordance with departmental standards and guidance.
13.Maintain effective communication across the multi-disciplinary team and with relatives where relevant. Attend multi-disciplinary team meetings.
14.Monitor, develop and maintain health and safety of self and others. Carry out risk assessments.
15.Develop own skills and professional development.
16.Take part in therapy team meetings and in-service training
