Position:Community Rehab Mental Health
Grade: Band 6
Hours: Full Time
Job Summary:

We would be looking for a motivated and enthusiastic person who ideally has had previous experience of working in a community mental health setting.
Duties would be to act as co - Care Coordinator to service users on the caseload, offering support to families or offering Carers assessment / gaining collateral information, completion of initial assessments with colleagues and covering duty system once duty induction completed.
